Why tension headaches keep returning

From an osteopathic perspective, a tension headache is rarely a problem of the head itself — it is often the visible end of a chain that starts lower: neck muscles held tight by screen posture, a stiff upper back, or restricted venous outflow from the head that builds pressure through the day.

According to the American Academy of Family Physicians, tension-type headache is the most common primary headache disorder, with a lifetime prevalence as high as 78%.

You know the story: it's 4 p.m., the screen in front of you blurs, and a tight band squeezes your forehead and temples — as if your head is in a vice. Painkillers take the edge off for a few hours, sleep helps until tomorrow, and then the band comes back. You blame stress, the weather, or "staring at the laptop too long" — but the pattern repeats week after week. Here's what osteopathy sees behind it: a tension headache often comes from muscles, not from the head itself — the thin scalp muscles stretched over the skull, the small muscles at the base of the skull, and the neck that carries your head's weight all day. When they stay tense for months, they may also disturb venous outflow from the head — and the pressure builds. Treating only the pain is like silencing a smoke alarm without looking for the fire. We look for where the tension starts.

Early warning signs your body sends

Osteopathy for tension headaches in Oakville

The body rarely "switches on" a headache out of nowhere — it usually whispers first. Pressure at the base of the skull after a workday. A sore scalp when you brush your hair or take off your glasses. Jaw tightness by evening — or noticing you're clenching your teeth. The urge to dig your fingers into your own neck, or to "crack" it yourself. Shoulders that feel like concrete by evening. A headache that painkillers no longer fully cover. That's not "just stress" and not "it'll pass on its own" — those are early warning signs that the muscles of your head and neck are stuck in tension. The good news: at this stage the body is easiest to help — the tension hasn't yet become its "normal". The sooner you respond to these early warning signs, the shorter the way back.

How classical osteopathy may help with tension headaches

Osteopathy for tension headaches in Oakville

Classical osteopathy looks at the whole chain behind a tension headache. In osteopathic practice, the upper cervical spine is the postural "key" of the body: tension here affects how the whole spine and shoulders hold themselves. During a visit we assess the muscles at the skull base, the scalp muscles, the neck and shoulders, and even the thoracic outlet area — when it's tight, venous outflow from the head may suffer. Then — gentle manual techniques: delicate work with tense muscles and stiff joints, with no sharp "cracking" or jerking. Many people are surprised at how light the touch is — yet the head often feels "freer" as the scalp and neck release. This approach may help ease muscle tension around the head and neck, support healthy circulation and mobility, and may support fewer, milder headache episodes over time. We don't promise to erase your headaches in one visit — we help the body gradually let go of long-held tension. Visits are led by osteopaths Dmytro and Natalia.

What your first 45-minute visit looks like

Your first visit lasts 45 minutes — like every appointment — and there's no rush. First, a conversation: when the headaches come, what they feel like, how long they've been happening, what you've already tried, what your typical day and sleep look like. Then — an observation: how you hold your head and shoulders, how your neck moves. Next — a careful hands-on assessment of the scalp muscles, the base of the skull, the neck and shoulders: where tissues are tense, where movement is limited. Please note: we do not diagnose medical conditions — we assess the state of soft tissues and mobility to understand what to work with using our hands. And already at the first meeting you get a clear plan: what we saw, what we'll work on, and how many visits it will most likely take.

How many visits will you need?

If the band around your head is visiting more often — don't wait until painkillers stop helping. Come in: we'll find where the tension is coming from and make a plan.

Frequently asked questions

Why does my headache always come back in the afternoon?

Tension accumulates through the day — hours of screen posture tighten the neck and scalp muscles, and by late afternoon the pressure reaches its peak. Painkillers calm the signal, but the pattern returns until the source of the tension is addressed.

Can tension headaches really come from the neck?

Very often, yes. The small muscles at the base of the skull and the neck that carries your head's weight all day are among the most common sources — a forward head position keeps them working overtime for hours.

How long is an appointment and how much does it cost?

Every appointment lasts 45 minutes and costs $140 CAD. First and follow-up visits cost the same.

What if my headaches turn out to be migraines, not tension headaches?

We don't diagnose medical conditions — we assess how the head, neck and upper back move and where tension is held. If we see signs that point beyond the musculoskeletal picture, we will tell you honestly and advise seeing your doctor.
